Information Recording and Program Registration
REGISTRATION:
Thank you to all who have expressed an interest in the PSW program. Registration for September 2024 will open on Monday, April 29th, and close on August 16th. Please note no late registrations will be accepted.
To register for the September 2024 course:
1.) Download the Mandatory Registration package found below. You will require the direct deposit page to upload as part of the registration.
2.) Click on this link: Register Here
3.) Watch your email for confirmation of enrollment and next steps.
4.) Contact your doctor to start on the medical documentation.
Please note the below documents are required for upload when applying (Your application will not be finalized with missing documents)
- Proof of Residency (Acceptable documents to verify address: Tenant Lease, Utility Bill, Property Tax Bill, Home Phone, Cable or Internet Bill)
- High School Transcript
- Direct Deposit form and Void Cheque (found on the last page of the Mandatory Registration Package)
