Proof of Canadian Residency: (Acceptable documents to verify Canadian residency: Canadian Birth Certificate, Canadian Permanent Residence Card, Canadian Passport, Refugee Protection Claimant Document, etc.)
Proof of Address: (Acceptable documents to verify address: Tenant Lease, Utility Bill, Property Tax Bill, Home Phone, Cable or Internet Bill) Ministry guidelines do not allow for a Driver's License to be used as proof of Address
