    PSW Registration Form September 2023

    Thank you for your interest in the CDSBEO PSW program. Please complete the form below to register for the CDSBEO September 2023  PSW Program.

    The cost of the program is $1,500.00. This amount is due in full prior to the start of the program. A $500.00 non-refundable deposit is required at time of registration to maintain your place in the program.
    Late fees will not be accepted unless it is approved in advance by the administration. A payment plan may be worked out with approval. 
    • Our preferred method of payment is debit VISA/Mastercard payments and credit cards.  You may send an e-transfer to [email protected] (Please provide your name and state "PSW deposit" in the comments) and email confirmation of transfer proof to [email protected]. 
    • You may also send an email to [email protected] to arrange a time to process payment over the phone.

    Once confirmation of payment is received, your application will be reviewed for completion and you will receive an email confirming your enrollment in the program.
