Thank you for your interest in the CDSBEO PSW program. Please complete the form below to register for the CDSBEO September 2022 PSW Program.

There is a $250 application fee to be placed on a class list. You are welcome to continue with your application, however your application will not be finalized until payment is received.

Making your application fee payment:

You may send an e-transfer to accountsreceivable@cdsbeo.on.ca (Please provide your name and PSW deposit in comments) and email confirmation of transfer proof to kristina.holden@cdsbeo.on.ca
You may also call 613-284-2613 ext. 3 to process payment via credit card over the phone. Phone lines are not currently answers daily - please leave a message and Kristina will contact you back. You may also send an email to kristina.holden@cdsbeo.on.ca to arrange a time to connect to process payment.

Once confirmation of payment is received, your application will be reviewed for completion and you will receive an email confirming your enrollment in the program.

Please note - the application fee will be refunded ONLY upon successful completion of module 1.

Proof of Residency (Acceptable documents to verify address: Tenant Lease, Utility Bill, Property Tax Bill, Home Phone, Cable or Internet Bill)
Proof of Canadian Citizenship (Acceptable documents include: Birth Certificate, Passport, Permanent Resident Card. )
Direct Deposit Form (CDSBEO requires direct deposit information to reimburse deposits paid, and eligible government placement stipend funding. Please provide your direct deposit form.)
